Traffic crashes are documented in one of two ways:
State Crash Forms:
These forms are completed by the investigating officer. They are used for crashes involving any type of injury and/or damages resulting in at least $1000. These forms can be obtained online by clicking here.
Driver Crash Forms (Blue Forms):
If you were involved in a minor crash that was not investigated by a law enforcement officer, you may submit a Driver Crash Report to the Texas Department of Public Safety using this printable form. This must be done within 10 days of the crash.
***NOTE***
SPD officers may investigate motor vehicle collisions that result in injury or death of a person or property damage to an apparent extent of at least $1,000 and file justifiable charges relating to the collision. Officers are not required to investigate collisions that occurred in privately owned parking areas or a privately owned parking lot where a fee is charged for parking or storing a vehicle.
